Under-Eye Filler

The tear trough is the groove that runs from the inner corner of the eye toward the cheek. Filler there can make a hollow look softer, but it is one of the more technical areas to treat. Not every under-eye concern is a hollow, so a clear conversation about what bothers you is important.

How it works

A small amount of soft hyaluronic acid gel is placed under the skin or just above the bone, often with a cannula. The gel fills the hollow and may reduce the shadow that makes the area look dark or tired. Dark circles caused by pigment or thin skin may not change much, and puffiness or fat bulges can look worse with filler. The doctor looks at the cause before deciding. The gel breaks down slowly over months, and it can be dissolved with an enzyme if needed.

Who it may suit

  • A hollow groove under the eyes that casts a shadow
  • A tired look linked to loss of volume in the upper cheek
  • People whose dark circles come mostly from shadow, not pigment
  • Those who want a modest, adjustable change

The treatment day

  1. Examine the under-eye area. The doctor checks for hollows, bags, pigment and skin thinness while you sit upright and look in different directions.
  2. Decide if filler fits. If another approach suits you better, the doctor should say so.
  3. Numbing and injection. A small amount of soft gel is placed slowly, often with a blunt cannula.
  4. Check and written advice. The result is checked in a mirror, and warning signs and aftercare are explained.

Aftercare

  • Do not press or rub the area for the first days.
  • Use a cold compress gently if the doctor advises it.
  • Sleep with your head slightly raised for a night or two.
  • Avoid intense exercise, saunas and alcohol for a day.
  • Call the clinic or seek urgent care for pain, vision changes or skin color changes near the eye.

Things to know first

  • Swelling and bruising are common, and swelling can linger longer here than in other areas.
  • Bluish tint (the Tyndall effect), lumps and puffiness can occur, especially with too much product.
  • Vascular occlusion is a rare but serious risk, and near the eye it can affect vision. Know the warning signs before you leave.
  • Not suitable for everyone, such as people with prominent bags, thin skin or active eye problems.
  • Not usually advised during pregnancy or breastfeeding. Results vary by person.

Talking through under-eye filler in English

Say what you notice: a dark shadow, a groove, puffiness, or color. Words like dark circle can mean very different things, so point to the exact area in a mirror. Ask the doctor to explain, in plain English, whether your concern is hollowness, pigment or fat, and whether filler is truly the right tool.

Ask which product is chosen for this area and why, how much will go in, and whether a cannula or needle will be used. Ask who is performing the injection. It is fair to ask, What are the warning signs, and what do I do if I notice them? Write down the answer and keep the clinic contact details with you.

Tell the doctor about past eye-area fillers, allergies, thyroid or eye conditions, and any blood-thinning medicine. Terms you may hear: tear trough, infraorbital hollow, malar, Tyndall effect and hyaluronidase. If a flight is near, ask how long bruising might show.

Under-Eye Filler: common questions

Will filler fix all dark circles?

No. It may help shadow caused by a hollow, but pigment and thin skin usually respond differently. The doctor should explain the cause first.

What are the warning signs after treatment?

Severe pain, white or mottled skin, or changes in vision need urgent care. Ask the clinic to list them in writing.

Can it be dissolved?

Hyaluronic acid gel can usually be dissolved with an enzyme. Confirm the type of filler before treatment.

How long will swelling last?

Often several days, but it can last longer here. The doctor can tell you what is typical for your case.
